Who was the registered owner at the time the vehicle was towed?

Provide the name, address and telephone number of the driver who parked the car, and any passengers or other witnesses present when the car was parked:

What company towed the vehicle?

What was the address where the vehicle was towed from?

Is this private or public property?

Is this part of a business area or mall?

If this was a business area, did you or anyone with you patronize the business? For how long? Did anyone make a purchase? If yes, do you have the receipt?

Before or after patronizing the business, did you leave the vehicle parked and go somewhere else? If yes: What time did you leave the vehicle, how long were you away from it, and what was your purpose for leaving?

Please provide a description of the exact space where the vehicle was parked prior to the tow:

Did you notice any signs posted warning about towing when you parked the car? If yes: Where was the sign posted? Did it include a local phone number for traffic enforcement Was there any additional information that you recall posted on the sign(s)?

What date and time did you notice the vehicle was towed or in the process of being towed?

How much time elapsed between parking and the time you noticed the vehicle was towed or in the process of being towed?

Did you see the vehicle being towed? If yes, can you describe the tow truck.

How many drivers where there? Did you get any names? Can you give a physical description of the driver(s)?

Did you talk to any tow truck employee before the car was removed? If yes:

Did you, or someone with you, advise that person(s) that you, or someone with you, was the owner/possessor of the vehicle?

Did you ask that person to release the vehicle? If yes, what was their reply?

Did that person(s) ask you, or someone with you, to pay money to have the vehicle released? If yes, was the vehicle still physically located in the private parking lot when the request for money was made?

How much money did they request? Did they advise you what types of payment they would accept? What else did they say?

Did you or anyone with you speak with any security person at the lot? Describe the person and the conversation.

Did you have a conversation about your vehicle being towed with any person at the business you were patronizing. If yes, please describe that person and the conversation. Don't forget to include occupation and title if you know it, (example, 7-11 Manager), and a physical description.

If you didn't see your vehicle being towed, how did you discover ii had been towed? Describe what you did after ii had been towed?

What was the date and time that you, or someone with you, first requested the tow company release the vehicle?

What was the name of the tow company, the person from whom the request was made (include a name and job title if you know it), and the location where the request was made?

Did you, or someone with you, ask anyone at the tow company for the reason why the vehicle was towed? If yes, what was the reason provided, and was any proof provided by the tow company?

If you were told you were parked in a fire lane, too close to a hydrant, or interfering with the entrance or exit, did the tow company provide you with a photograph of your vehicle depicting where it was parked prior to the tow?

Were you told that a person from a business or a property company requested the tow? If yes, specify what information you were provided, like the name of the person authorizing the tow.

Did you later try to contact the person who allegedly requested the tow? If yes, describe all the details of that conversation.

Did you or someone with you eventually pay money to have the vehicle released? If yes, who requested the payment be made? (Describe the person if you don't know the name.)

How much money did that person request be paid?

Were you informed how the amount was calculated? If yes, describe:

What was the method of payment, (Check, cash, credit card)?

Did you, or someone with you, ask any person if you could pay by credit card? If yes, what was the response?

Did any person from the towing company inform you, or the person with you that they would not accept payment by credit card? If yes, what person(s) told you this?

Did you, or someone with you, make more than one trip to the tow yard before the vehicle was released? If yes, please explain what happened.

Was the vehicle damaged in any way? If yes, did you get an estimate for the damage? (Submit a copy)

Did you pay for the repair? If yes, how much? (Submit a copy of the receipt)

Please attach COPIES of any documents you have regarding this matter, including photographs, letters, and receipts.
